Abstract
Cilj ovog rada je prikazati aktivnosti medicinske sestre kod oboljelih od hipotireoze i hipertireoze. Njezina aktivnost i važnost se najbolje iščitava iz utvrđivanja samih potreba, planiranja, provođenja i konačno evaluacije. Ona kao zdravstveni djelatnik pridonosi oporavku bolesnika svojim znanjem i kompetencijama. Cilj je prikazati način na koji se utvrđuje potreba za zdravstvenoj njezi u Sjedinjenim Američkim Državama te usporediti sa načinom na koji se utvrđuje u Republici Hrvatskoj. Usporedba se napravila između sestrinskih dijagnoza koje koriste obje države kod zbrinjavanja oboljelih od hipotireoze i hipertireoze. Pretraživanjem literature pronađeni su podaci koji ukazuju na hipotireozu i hipertireozu kao javne zdravstvene probleme. Broj hospitalizacija u Hrvatskoj tijekom 2016. godine prema podacima Hrvatskog zavoda za javno zdravstvo zbog ostalih hipotireoza je bilo 279. Broj hospitalizacija u Hrvatskoj tijekom 2016. godine prema podacima Hrvatskog zavoda za javno zdravstvo zbog tireotoksikoze odnosno hipertireoze bilo je 1035 oboljelih uključujući sve dobne skupine. U člancima na internetskim stranicama poput poput „Quia“, „RNpedia“ te „Nurselabs“ pronađene su informacije o sestrinskim dijagnozama u Sjedinjenim Američkim Državama. Sestrinske dijagnoze u Republici Hrvatskoj su preuzete sa stranica Hrvatske komore medicinskih sestara. Uspoređivanjem sestrinskih dijagnoza u našoj državi koje su odobrene od strane Hrvatske komore medicinskih sestara i onih koji se koriste u Sjedinjenim Američkim Državama uočene su razlike. Sestrinska dijagnoza koju ne upotrebljavaju naše medicinske sestre u zdravstvenoj njezi oboljelih od hipotireoze je „Smanjeni srčani učinak u/s promijenjenim brojem otkucajem srca i ritma uslijed smanjenog metabolizma miokarda“ najbolje pokazuje više ovlasti i kompetencije kod medicinskih sestara u Sjedinjenim Američkim Državama. Nadalje, procjena stanja štitnjače auskultacijom i palpacijom te intervencije kojima medicinske sestre auskultiraju zvukove srca i pluća veće su kompetencije kod njihovih u odnosu na medicinske sestre u našoj državi. Uočena je razlika u samom pristupu procesu zdravstvene njege tako se kod nas koriste 4 faze, a u SAD-u 5 faza u procesu zdravstvene njege oboljelih. Pokazana je aktivnosti medicinske sestre kod oboljelih od hipotireoze i hipertireoze. Njezina aktivnost i važnost se najbolje iščitava iz utvrđivanja samih potreba, planiranja, provođenja i konačno evaluacije.
Abstract (english)
The aim of this paper is to present the activities of a nurse in patients with hypothyroidism and hyperthyroidism. Its role and importance is best reflected in identifying the needs themselves, planning, implementing and ultimately evaluating them. As a healthcare professional, she contributes to the patient's recovery with her knowledge and competencies. The goal is to show the way it determines the need for health care in the United States and to compare the way in which it is determined in the Republic of Croatia. A comparison was made between the nursing diagnoses used by both countries in the management of patients with hypothyroidism and hyperthyroidism. A search of the literature found data indicating hypothyroidism and hyperthyroidism as public health problems. The number of hospitalizations in Croatia during 2016 according to the data of the Croatian Institute of Public Health for other hypothyroidism was 279. The number of hospitalizations in Croatia during 2016 according to the data of the Croatian Institute of Public Health for thyrotoxicosis and hyperthyroidism was 1035 patients, including all age groups. Articles on websites such as Quia, RNpedia and Nurselabs have found information about nursing diagnoses in the United States. Nursing diagnoses in the Republic of Croatia were taken from the pages of the Croatian Nursing Chamber. Comparing nursing diagnoses in our country that are approved by the Croatian Chamber of Nurses and those used in the United States, there are differences. A nursing diagnosis not used by our nurses in the care of patients with hypothyroidism is "Reduced cardiac output in / with altered heart rate and rhythm due to decreased myocardial metabolism" best demonstrates more authority and competence in nurses in the United States. Furthermore, the evaluation of thyroid conditions by auscultation and palpation and the interventions by which nurses auscultate the sounds of the heart and lungs are more competent in theirs than the nurses in our country. There is a difference in the approach to the health care process, so that in our country 4 steps are used, and in the USA there are 5 stages in the process of health care of the sick. Nurse activity in patients with hypothyroidism and hyperthyroidism has been demonstrated. Its role and importance is best reflected in identifying the needs themselves, planning, implementing and ultimately evaluating them.
