Abstract Gnojni meningitis je prije izuma antibiotika bio smrtonosna upala moždanih ovojnica. Uzrokuju ga različite bakterije koje se dokazuju u krvi oboljelog ili cerebrospinalnom likvoru. Krv se uzorkuje u specifične sterilne setove sa podlogama tj bujonima. Postupak se naziva hemokultura, Čak i prije izoliranja uzročnika pristupa se antimikrobnom liječenju, ali je nerijetko potrebno i supurativno liječenje. Likvor se dobije postupkom lumbalne punkcije koju izvodi liječnik u aseptičnim uvjetima. Zadaća medicinske sestre je da uzorak pravovremeno dostavi u obližnji laboratorij s potrebnom medicinskom dokumentacijom. Oboljeli se liječe u jedinicama intenzivne skrbi, a sam oporavak traje dugo jer bolest uzrokuje neurološke ispade. Incidencija meningokokne bolesti je 0.5-5/10000 stanovnika, a bolest se češće javlja u proljeće u područjima umjerene klime. Zdravstvena njega oboljelog od gnojnog meningitisa je specifična jer na samom početku pri prijemu medicinska sestra treba strogo voditi računa o higijeni ruku, eventualnoj izolaciji bolesnika, a kasnije i o radu sa obitelji oboljelog jer je oporavak dugačak. Sestrinske dijagnoze koje se često javljaju kod oboljelih od gnojnog meningitisa su: hipertermija, bol, anksiozniznost i visok rizik za hipertermiju. Gnojni meningitis i njegove komplikacije, a osobito meningokokna sepsa predstavljaju po život zaraženog opasno stanje na koje treba što brže reagirati adekvatnom skrbi i liječenjem,ali i profilaktički zaštititi sve osobe koje su na bilo koji način našle u kontaktu oboljelog. Opisana sestrinska skrb omogućava oboljelima i članovima njihove obitelji da strpljivo prođu kroz proces rehabilitacije i nauče se određenim obrascima ponašanja u slučaju nastalih trajnih oštećenja kod oboljelog. Kako bi sestrinska skrb bila što efikasnija, sestra mora sagledati sve obrasce zdravstvenog funkcioniranja, a za ono područje u kojem se javlja problem izraditi specifičan plan zdravstvene njege.
Abstract (english) Purulent meningitis was a deadly meningitis before the invention of antibiotics. It is caused by various bacteria that are detected in the patient's blood or cerebrospinal fluid. Blood is sampled in specific sterile sets with substrates, ie. broths. The procedure is called blood culture. Even before the causative agent is isolated, antimicrobial treatment is approached, but suppurative treatment is often required. The cerebrospinal fluid is obtained by a lumbar puncture performed by a physician under aseptic conditions. The task of the nurse is to deliver the sample in a timely manner to a nearby laboratory with the necessary medical documentation. Patients are treated in intensive care units, and the recovery itself takes a long time because the disease causes neurological outbursts. The incidence of meningococcal disease is 0.5-5 / 10,000 inhabitants, and the disease occurs more frequently in the spring in temperate climates. The health care of a patient with purulent meningitis is specific because at the very beginning the nurse should strictly take care of hand hygiene, possible isolation of the patient, and later work with the patient's family because the recovery is long. Nursing diagnoses that often occur in patients with purulent meningitis are: hyperthermia, pain, anxiety, and a high risk of hyperthermia. Purulent meningitis and its complications, and especially meningococcal sepsis, represent a life-threatening condition that should be responded to as quickly as possible with adequate care and treatment, but also prophylactically protect all persons who have come into contact with the patient in any way. The described nursing care allows patients and their family members to patiently go through the rehabilitation process and learn certain patterns of behavior in case of permanent damage to the patient. In order for nursing care to be as effective as possible, the nurse must consider all patterns of health functioning, and for the area where the problem occurs, develop a specific health care plan.
