Abstract
Disanje je osnovna životna funkcija, a dišni sustav omogućuje izmjenu respiracijskih plinova. Traheotomija je kirurški postupak kojim se pravi otvor (traheostoma) na prednjoj stijenci vratnoga dijela dušnika. Izvodi se u razini drugoga ili trećega trahealnoga hrskavičnoga prstena i drži se otvoren umetanjem trahealne kanile Traheotomijom se zaobilazi mjesto poremećaja disanja, omogućuje čišćenje donjih dišnih putova od sekreta, sprječava se aspiracija, smanjuje se anatomski mrtvi prostor, kreira se mjesto za provođenje endotrahealne anestezije. Traheostoma može biti privremena i trajna. Traheotomija može biti hitan ili elektivan (planirani) kirurški zahvat. Dekaniliranje je spontano zatvaranje traheostome. Odluka o dekanilaciji ovisi o tijeku bolesti i ponovnoj uspostavi prohodnosti dišnog puta. Dugoročni cilj i želja je humanim pristupom poboljšati kvalitetu života bolesne djece i njihovih obitelji. Otpust iz bolnice određen je roditeljskom spremnošću nakon medicinske procjene od strane liječnika i medicinske sestre. Boravak djeteta u obiteljskom domu krajnji je cilj u skrbi za traheotomirano dijete. Život u obiteljskom okruženju uz sve poteškoće organizira najkalitetniji život i najveću razinu sreće za dijete i njegovu obitelj.
Abstract (english)
Breathing is a basic life function, and the respiratory system allows exchange of respiratory gases. A tracheotomy is a surgical procedure in which a doctor makes an opening (tracheostomy) in the front wall of the cervical part of the trachea. It is performed in the projection of the second or third tracheal cartilage ring and is held open by inserting a tracheal cannula. Tracheotomy helps us to bypass a place of disordered breathing, allows cleaning of the lower respiratory tract secretions, prevents aspiration, reduces the anatomic dead space, creates a place for implementation of endotracheal anesthesia. Tracheostomy can be temporary or permanent. Tracheotomy may be an urgent or a planned surgical operation. Removing the cannula spontaneously closes the tracheostomy. It depends on the course of the disease and the restoration of patency of the tracheobronhial tree. The long-term goal and desire is humane approach to improve the quality of life of sick children and their families. Going home is decided on a case by case basis, when doctors and nurses assess the readiness and when the parent feels ready and safe for self-care of their child.
