Abstract UVOD: Autoimune bolesti su kronične bolesti obilježene čitavim spektrom kliničkih manifestacija. Za posljedicu imaju brojna oštećenja uz smanjenje sposobnost pojedinca u obavljanju uobičajenih aktivnosti. Upravo zbog kroničnog tijeka i nerijetko trajnih oštećenja, ove bolesti direktno utječu na kvalitetu života oboljelih.
CILJ: Procijeniti kvalitetu života bolesnika oboljelih od RA, SLE-a i SS-a , zatim ispitati različitosti između pojedinih skupina, kao i odnos HRQOL s obzirom na dužinu trajanja bolesti, starost te socio-ekonomske čimbenike.
ISPITANICI I METODE: U studiju su uključena 94 bolesnika (RA 32, SLE 32, SS 30) koja zadovoljavaju ACR kriterije za RA, SLICC kriterije za SLE i SICCA kriterije za SS. Kvaliteta života je ispitivana korištenjema SF-36 upitnika koji je nadopunjen podatcima o vrsti bolesti, spolu, dužini trajanja bolesti i socio-ekonomskim podatcima.
REZULTATI: Bolesnici s RA imaju značajno lošiju kvalitetu života u domeni fizičkog funkcioniranja i tjelesne boli prema upitniku SF-36 u odnosu na oboljele od SLE-a (p=0.005, p= 0.036), dok nije utvrđena značajna razlika u odnosu na SS. U oboljelih od SLE je utvrđena značajna povezanost starosne dobi s kvalitetom života u svim domenama SF-36 upitnika, dok je duljina trajanja bolesti značajno povezana s fizičkim funkcioniranjem (p=0,048), vitalnošću i energijom (p=0.038) te socijalnim funkcioniranjem (p=0.008). Dužina trajanja bolesti nije povezana kvalitetom života u bolesnika s RA i SS-om. Dob bolesnika je negativno povezana s percepcijom općeg zdravlja (p=0.015) u SS-u. Kvaliteta života u domeni fizičkog funkcioniranja je bolja u bolesnika većeg stupnja obrazovanja (p=0.042). Najveću vrijednost u HRQOL su iskazali samci, a najmanje su vrijednosti iskazali udovci/udovice. Nije potvrđena povezanost ukupnih mjesečnih prihoda i radnog statusa s kvalitetom života.
ZAKLJUČAK: Naše istraživanje potvrđuje da bolesnici s RA imaju lošiju HRQOL u odnosu na bolesnike sa SLE i SS. Bolesnici starije dobi imaju lošiji HRQOL u SS-u i SLE-u. Trajanje bolesti negativno utječe na kvalitetu života u SLE-u. U sve tri skupine potvrđena je povezanost s bračnim statusom pri čemu se najlošija kvaliteta života pokazala u udovica/udovaca, a najbolja u bolesnika koji žive sami. Također je utvrđeno da je bolja kvaliteta života u bolesnika s većim stupnjem obrazovanja. Ukupni mjesečni prihodi i radni status nije utjecao na kvalitetu života.
Rutinska procjena HRQOL se preporučuje za otkrivanje i praćenje utjecaja bolesti na različite aspekte nihova života.
Abstract (english) INTRODUCTION: Autoimmune diseases are chronic diseases marked by a wide spectrum of clinical manifestations. As a result, they have numerous damages in reducing the individual's ability in performing of everyday activities.Precisely because of the chronic course and often the irreversible damage, these diseases directly affect the quality of life of the patients.
OBJECTIVE: The purpose of this study was to assess the quality of life of patients suffering from RA, SLE and SS, then examine the differences between certain groups, as well as the HRQOL relationship given the duration of the disease, age and socio-economic factors.
SUBJECTS AND METHODS: The study included 94 patients (RA 32, SLE 32, SS 30) who meet ACR criteria for RA, SLICC criteria for SLE and SICCA criteria for SS. The quality of life was examined by using the SF-36 questionnaire which was supplemented by data on the type of disease, gender, the duration of the disease and socio-economic data.
RESULTS: Patients with RA have a significantly lower quality of life in the domain of physical functioning and physical pain according to the SF-36 questionnaire in relation to SLE patients (p = 0.005, p = 0.036) while no significant difference has been established compared to SS. In patients with SLE, a significant age-related association with the quality of life in every domain of SF-36 questionnaire was established while the duration of the disease was considerably associated with physical functioning (p = 0.048), vitality and energy (p = 0.038) and with social functioning (p = 0.008). The duration of the disease is not associated with the quality of life in patients with RA and SS. The patients’ age is negatively related to the perception of general health (p = 0.015) in SS.The quality of life in the domain of physical functioning is better in patients with higher education (p = 0.042). The highest values in HRQOL were expressed by singles while the lowest values were expressed by widowers / widows. No connection between thetotal monthly income and employment status with the quality of life is confirmed.
CONCLUSION: Our research confirms that patients with RA have poorer HRQOL in relation to patients with SLE and SS. Elderly patients have poorer HRQOL in SS and SLE. The duration of the disease has a negative impact on the quality of life in SLE. In all three groups a connection with marital status was confirmed, where the worst quality of life has been shown with widows / widowers, and the best in patients who live alone.It has also been established that the quality of life is better in patients with a higher level of education. The total monthly income and employment status did not affect the quality of life. A routine assessment of HRQOL is recommended for detecting and monitoring the impact of disease on various aspects of life.
