Abstract Cilj rada: Cilj rada bio je ispitati zastupljenost broja Kell imunizacija u odnosu na ukupan broj imunizacija kod trudnica. Uspoređena je zastupljenost anti-Kell protutijela u populaciji trudnica i ambulantnoj predoperativnoj populaciji. Uspoređen je broj trudnica s anti-Kell protutijelima na prvom i kontrolnom testiranju.
Izvori podataka i metode: Podaci za ovo istraživanje prikupljeni su iz računalnog programa e-Delphyn te se odnose na ispitanike testirane u Centru za transfuzijsku medicinu u Splitu u period od početka 2014. do 6. svibnja 2018. godine. Imunohematološko ispitivanje podrazumijeva određivanje eritrocitne krvne grupe i test pretraživanja antieritrocitnih protutijela. Test pretraživanja antieritrocitnih protutijela se izvodi u indirektnom antiglobulinskom testu, a ako je test pozitivan, određuje se specifičnost te kod trudnica titar protutijela.
Rezultati: U praćenom razdoblju ukupan broj identificiranih protutijela bio je 1372, od kojih su 145 (10,57%) bila anti-Kell protutijela. Od 145 slučajeva Kell imunizacija, 141 (97,24%) bilo je s anti-K i 4 (2,76%) s anti-Kpa protutijelima. U ambulantnoj predoperativnoj populaciji bilo je 117 slučajeva (80,69%) Kell imunizacija, od kojih 113 (96,58%) s anti-K i 4 (3,42%) s anti-Kpa protutijelima. Od ukupno 702 identificirana protutijela kod trudnica, kod 28 (3,99%) trudnica pronađena su anti-K protutijela. Od 28 trudnica s anti-K protutijelima, kod 2 (7,14%) trudnice anti-K protutijela su novootkrivena na prvom testiranju. U 26 (92,86%) trudnica anti-K protutijela su potvrđena na kontrolnom testiranju.
Zaključci: Što se tiče populacije trudnica, samo su dvije trudnice s anti-K protutijelima bile novootkrivene u praćenom razdoblju, a ostalih 26 su potvrđene na kontrolnom testiranju. Značajno mali broj imuniziranih trudnica s anti-K protutijelima u odnosu na ostala otkrivena antieritrocitna protutijela kod ispitanika posljedica su transfuzijskog liječenja žena u generativnoj dobi Kell negativnim koncentratima eritrocita.
Abstract (english) Objective: The aim of the study was to examine frequency of Kell immunizations in relation to the total number of immunizations in pregnant women. The frequency of anti-Kell antibodies in the pregnant women and preoperative outpatients was compared. The number of pregnant women with anti-Kell antibodies and preoperative outpatiens was compared. The number of pregnant women with anti-Kell antibodies at first and control testing was compared.
Data sources and methods: The data for this study were collected from the e-Delphyn computer program and they are related to examinees tested at the Center for Transfusion Medicine in Split from the beginning of 2014 to May 6, 2018. Immunohematological testing involves the determination of erythrocyte blood groups red blood cell antibody screen. The red blood cell antibody screen is done in an indirect antiglobuline test and if this test is positive, the specificity of this antibody is determined and in pregnant women titer of antibodies as well.
Results: There were 1372 identified antierythrocyte antibodies during the follow-up period, of which 145 (10,57%) were anti-Kell antibodies. Of 145 Kell immunizations, 141 (97.24%) were with anti-K and 4 (2.76%) with anti-Kpa antibodies. There were 117 (80,69%) cases of Kell immunizations in the preoperative outpatients, of which 113 (96.58%) with anti-K and 4 (3.42%) with anti-Kpa antibodies. Out of total of 702 antibodies identified in pregnant women, 28 (3.99%) were anti-K antibodies. Of 28 pregnant women with anti-K antibodies, in 2 (7.14%) anti-K antibodies were newly detected at the first testing. In 26 (92.86%) pregnant women, anti-K antibodies were confirmed at the control testing.
Conclusions : Only two pregnant women with anti-K antibodies were newly detected at the first testing in the follow-up period, and the other 26 were confirmed at control testing. Significantly small number od immunized pregnant women with anti-K antibodies compared to other detected anti-erythrocyte antibodies in the examinees is due to treating women of generative age with Kell negative erythrocyte concentrates.
