Abstract
Rak pluća najčešći je uzrok smrti povezan s pojavom raka u svijetu. U vrijeme dijagnoze, rak pluća često je već u naprednoj fazi. Opservacijska ispitivanja kod visokorizične populacije pokazala su da detekcija raka pluća probirnom nisko doznom spiralnom tomografijom (LDCT) detektira više karcinoma pluća nego detekcija rendgenskom snimkom pluća. Daljnja randomizirana ispitivanja potrebna su za uspoređivanje smrtnosti specifičnoj za bolest između skenirane i neopažene populacije. Kada bolesnik prvi put počne program probira LDCT-om za rak pluća, dijagnosticirani rak pluća može biti u uznapredovaloj fazi, iako je bolesnik asimptomatski. Optimiziranje ciljne populacije za probir raka pluća ima ogroman značaj, ne samo za pojedince koji teže riziku i prednostima, nego i za zdravstvene djelatnike, zdravstveno osiguranje i vladu koja moraju razmotriti fiskalni učinak provođenja probira raka pluća. Hipoteza NELSON studije je da probirom raka pluća nisko doznim spiralnim CT-om se može smanjiti 10-godišnja smrtnost raka pluća za 25% kod visoko rizičnih pušaća u razdoblju od 50 do 75 godine starosti. NLST i NELSON studije uključile su dva bitna rizik za rak pluća: godine i povijest pušenja. Moguće je da s boljim prognozama rizika, probir može biti učinkovitiji, s više dijagnosticiranih karcinoma pluća i manjim rizikom izlaganja štetnom ionizirajućem zračenja kod pojedinaca.
Abstract (english)
Lung cancer is the most common cause of death, related in cancer worldwide. At the time of diagnosis, lung cancer is often already at an advanced stage. Observational studies in a high-risk population showed that detection of lung cancer by low-dose helical tomography (LDCT) screening detected more lung cancer than lung X-ray detection. Further randomized trials are needed to compare disease, specific mortality between the scanned and unnoticed populations. When a patient first begins a LDCT screening program for lung cancer, the diagnosed lung cancer may be at an advanced stage, although the patient is asymptomatic. Optimizing the target population for lung cancer screening is of tremendous importance, not only for individuals at risk and benefits, but also for healthcare professionals, health insurance and government, who must consider the fiscal impact of conducting lung cancer screening. The NELSON study hypothesis is that lung cancer screening with low-dose helical CT can reduce 10-year lung cancer mortality by 25% in high-risk smokers between 50 and 75 years of age. NLST and NELSON studies have included two significant risks for lung cancer: age and smoking history. It is possible that with better risk forecasts, screening may be more effective, with more diagnosed lung cancers and less risk of exposure to harmful ionizing radiation in individuals.
