Abstract
U radu su opisane epidemiološke karakteristike tuberkuloze na području Splitsko-dalmatinske županije. Analizom incidencije oboljelih od tuberkuloze na području Splitsko-dalmatinske županije pokazuju tendenciju pada s laganim porastom u zadnje dvije godine. Usporedba dobnih struktura oboljelih na području Splitsko-dalmatinske županije ukazuje da su najveće stope incidencije zabilježene u najstarijim dobnim skupinama 65 godina i više, po tome dobna raspodjela incidencije oboljelih na području Splitsko-dalmatinske županije odgovara onoj razvijenih zemalja svijeta i Hrvatske. Muškarci gotovo dvostruko češće obolijevaju od tuberkuloze u odnosu na žene. U promatranom desetogodišnjem razdoblju nije bilo ni jednog slučaja tuberkuloznog meningitisa u djece, što je dobar pokazatelj uspješnog cijepljenja djece. Od ukupno 645 oboljelih od tuberkuloze, 456 oboljelih tj. 75,34% su imali pozitivnu kulturu iskašljaja ili aspirata bronha. Gotovo polovica bolesnika plućnom tuberkulozom je imala mikroskopski pozitivan sputum. Rezistencija na antituberkulotike je u Splitsko-dalmatinskoj županije vrlo rijetka, oko 1,44%. Glavne prijetnje globalnom nadzoru nad tuberkulozom u 21. stoljeću su pandemija infekcije HIV-om, tuberkuloza praćena različitim kroničnim bolestima, rizičnim životnim navikama (alkoholizam, pušenje, intravensko uzimanje droga) te pojava tuberkuloze uzrokovane sve rezistentnijim sojevima mikobakterija koji je čine teško izlječivom bolešću, to problemi s kojima se u nadzoru nad tuberkulozom bore mnoge zemlje, a u Hrvatskoj i u Splitsko-dalmatinskoj županiji se uspješno drže pod kontrolom. Iako je epidemiološka situacija vezano uz tuberkulozu u Splitsko-dalmtinskoj županiji općenito povoljna, treba intenzivno nastaviti raditi na provođenju i jačanju mjera suzbijanja i sprječavanja tuberkuloze kako bismo tuberkulozu eliminirali kao javnozdravstveni problem.
Abstract (english)
This work describes the epidemiological characteristics of tuberculosis in Split-Dalmatia county. The analysis of the incidence of tuberculosis in the area of Split-Dalmatia county showed a slight tendency to fall. Comparison of age structure of the patients in Split-Dalmatia county indicates that the highest incidence rates were observed in the oldest age grous of 65 and more, in that the age distribution of the incidence of patients in Split-Dalmatia county corresponds to that of the developed countries of the world and Croatian. The men almost twice as likely to suffer from tuberculosis than women. In the same ten-year period, there were no cases of tuberculous meningitis in children, which is a good indicator of a successful vaccination of children. Almost half of the patients had pulmonary tuberculosis microscopically positive sputum. Resistance to anti-tuberculosis drugs in the Split-Dalmatia county is very rare, about 1,44%. The main threats to the global control of tuberculosis in the 21st century are the pandemic of HIV infection, tuberculosis accompanied by a variety of chronic diseases, risky lifestyle (alcoholism, smoking, intravenous drug use), and emergence of tuberculosis caused by more resistant strains of mycobacteria that make it difficult treatable disease, problems which many countries is fighting with, but in Croatia and Split-Dalmatia county is successfully kept under control. Although the epidemiological situation regarding tuberculosis in the Split-Dalmatia county is generally favorable we should continue to intensively work on the implementation and strengthening of measures to control and prevent tuberculosis in order to eliminated it as a public health problem.
