Abstract
Jetrene bolesti povezane s trudnoćom pogađaju do 3% trudnica te mogu biti povezane sa značajnim morbiditetom i mortalitetom majke i novorođenčeta (1). Bolesti jetre kod trudnica mogu se klasificirati u one u ranoj trudnoći: trudnička hiperemeza (lat. hiperemiagravidarum - HG) i one u kasnoj trudnoći (AFLP): preeklampsija s zahvaćenom jetrom uključujući hemolizu, povišeni jetreni enzimi i sindrom niskog broja trombocita (HELLP), jetrena ruptura/infarkt te intrahepatična kolestaza trudnoće (ICP). Liječenje i primaljska skrb trudnice sa jetrenom bolesti je nešto s čim se učestalo susrećemo, a vrlo je zahtjevna s obzirom da prije nego se odlučimo na bilo koji postupak u liječenju i/ili skrbi trebamo uzeti obzir ne samo dobrobit majke već i dobrobit nerođenog djeteta. Trudnoća je stanje u kojem se u tijelu žene događa velik broj fizioloških promjena i prilagodbi tom specifičnom stanju pa je kod dijagnostičke obrade trudnica u slučaju jetrene disfunkcije potrebno uzeti u obzir opisane fiziološke promjene te potrebu za minimalizacijom izloženosti zračenju. Kod cilja rada stavljen je naglasak na primaljsku skrb u zbrinjavanju trudnice s jetrenim bolestima u trudnoći te prikazan cjelovit pristup trudnici s jetrenim bolestima u trudnoći te same specifičnosti zdravstvene njege. Kao što je već spomenuto jetrene bolesti su povezane sa značajnim morbiditetom i mortalitetom za majku i novorođenče stoga je osnovni cilj antenatalne zaštite smanjenje preinatalnog morbiditeta i mortaliteta, te maternalnog morbiditeta i mortaliteta. Antenatalna zaštita se provodi na svim razinama zdravstvene zaštite. Priprema trudnice za tijek trudnoće i sam porod uključuje timski rad liječnika i primalje, kao i primarne i sekundarne zdravstvene razine. Apsolutni naglasak ide na zdravstveni odgoj kao jedan od najvažnijih faktora (uz redovite preporučene ginekološke preglede) ranog otkrivanja jetrenih bolesti kao i prevencije istih. Zdravstveni odgoj primalja provodi na svim razinama zdravstvene zaštite.
Abstract (english)
Pregnancy relatedliverdiseasesaffectup to 3% of pregnant women and they are causingsignificantmorbidity and mortality for mother and infant (1). They are classified as early pregnancy disease: hyperremesisgravidarum (HG) and late pregnancy liverdisease: preeclampsia with the affectedliverincludinghemolysis, elevatedliverenzymes and lowplatele syndrome (HELLP), liver rupture/infraction and intrahepaticcholestasisog pregnancy (ICP). The Treatment and midwifery care of a pregnentwoman with leverdisease is somethingwe are otfenfaced with but the chalangeliesint he need to consider the safety of both the expectantmother and the unborn fetus int heclinicalmanagmentdecision. Pregnancy is a condition in which a largenumber of physiologicalchanges and adaptations to thisspecificcontidionoccurint he body of a women, sopsysiologicalchanges and the needs to minimizeradiationexposureshouldconsiderint he diagnostictreatment of pregnant womenint he case of liverdisfunction. The goalwas to show complebity of midwifery care of a pregnant woman with liverdisease and to show a holisticapproach and the specifics of health care. As alreadymentioned, liverdiseases are associated with significantmobility and mortality (mother and child) so the primarygoal of antenatal care is to reduceprenatal and maretnalmorbidity and mortality. The preparationoft he pregnant woman for pregnancy and childbirthitselfinvolves the teamwork of doctors and midwives, as well as primary and secondarylevels in addition to regularcontrols, the psychologivalpreparationoft he pregnant woman for pregnancy and delivery of health baby. Keypoint is on health educationwhat is one of the most impertantfactors (along with regulargynecologicalexaminations) for the earlydetection and prevention of liverdiseases. Midwifery health education is carried put at alllevels of healt care.
